About the Role

We are seeking an enthusiastic and motivated Advanced Clinical Pharmacist to join our team at South West Yorkshire Partnership NHS Trust. As a key member of our primary care mental health pharmacy team, you will play a vital role in providing specialist mental health clinical pharmacy services to GP practices and service users.

The successful candidate will work alongside advanced clinical pharmacists and medicines optimisation technicians to improve access to information on mental health medicines and support appropriate prescribing and safe use of medicines. You will also work closely with Kirklees Primary Care Networks, liaising with both primary care and secondary care colleagues to support those with mental health conditions being managed in primary care.

Key Responsibilities
  • Provide direct patient care, carrying out patient consultations and medication reviews for mental health medicines.
  • Rationalise and optimise medicines for mental health conditions.
  • Support other healthcare professionals in relation to mental health medicines, including responding to queries and education and training.
  • Work closely with other members of the team to support medicines reconciliation on discharge from mental health services, shared care management, and overprescribing medication reviews.
