Position: Community Adult Social Care Practitioner

Team: Community Discharge to Assess

Contract Type: Ongoing

Working Hours: 37 hours per week with hybrid working options

An experienced Community Adult Social Care Practitioner is sought to join our dedicated Community Discharge to Assess Team. In this pivotal role, you will be tasked with performing thorough assessments under the Care Act for individuals transitioning from hospital to temporary care placements.

Your main focus will be to evaluate the care requirements of each individual, ensuring the identification of suitable services and the coordination of necessary support.

This position demands a proactive approach, as timely and high-quality assessments are essential. As an integral member of our social care team, you will significantly contribute to empowering individuals to reach their personal goals.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals discharged from healthcare facilities, determining their community support needs.
  • Evaluate the physical, mental, and emotional well-being of clients, alongside their functional capabilities, to ascertain the appropriate level of care.
  • Engage collaboratively with clients, their families, and multidisciplinary teams to collect pertinent information and formulate detailed care plans.
  • Offer guidance, support, and emotional assistance to clients and their families, aiding them in understanding their choices and making informed decisions.
  • Coordinate with community resources, including healthcare providers and voluntary organizations, to facilitate the effective delivery of services.
  • Maintain precise and current documentation of assessments, interventions, and outcomes, adhering to organizational and legal standards.
  • Stay informed about legislative changes, policies, and best practices relevant to social work and community support services.

Qualifications:

  • DIPSW or equivalent qualification in Social Work.
  • Registered with Social Work England.
  • Possess an enhanced and current DBS certificate.
  • A minimum of one year of post-qualification experience in social work.
  • Able to work independently as well as collaboratively within a team and multi-agency environments.
  • Exhibit flexibility in work approach and hours as required.
