Social Care Advisor

About the Role

The One Front Door service is the single point of contact for all of Warrington residents needing to access Warrington's Intermediate Tier services. As a Social Care Advisor, you will be the first point of contact for all new referrals coming into Adult Social Care, the reporting of Safeguarding Adult Concerns, and the Local Authority's duty for the provision of the AMHP service to coordinate assessments under the Mental Health Act (1983 as amended).

Key Responsibilities

  • Converse with callers, both professionals and members of the public, using advanced contact handling skills to question, listen, and understand the root cause for their call and support them to access the right support, from the right team, at the right time.
  • Ensure all initial assessments for adults and consequent service provision meet the Council's statutory responsibilities, including the responsibility to promote an individual's well-being.
  • Manage the demand into Adult Social Care, promoting Information, Advice, and Guidance, and ensuring all contacts and assessments fully utilise the Strengths-Based Approach model to Adult Social Care and to engage in 'good conversations' with Warrington people.
  • Work closely with other professional, voluntary, and non-statutory agencies offering information, advice, and guidance, including signposting to alternative community-based provision, for those referrals that are not eligible for adult social care as defined by the Care Act 2014.
