About the Role

We are seeking a highly motivated and experienced Care Coordinator to join our team at Essex Partnership University NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals experiencing a First Episode of Psychosis.

Key Responsibilities
  • Undertake care co-ordination of a caseload of clients, ensuring that their needs are assessed, planned, and implemented in a timely and effective manner.
  • Carry responsibility for the assessment of care needs, the development, implementation, and evaluation of recovery-based care plans.
  • Participate in the collection of data, including entry and exit pathways to care, duration of untreated psychosis, and outcome measures.
  • Maintain effective individualized care through assessment of service user and carer problems, planning, implementing, and evaluating interventions, and documenting individualized care.
  • Undertake initial health and social needs assessments and participate in multi-disciplinary team meetings to look at appropriate selection and allocation of clients to CPA and regular reviews.
  • Plan creatively to meet the needs of clients, including the provision of evidence-based interventions.
  • Prioritize and manage workload to ensure the effective management of a defined group of clients with complex needs.
  • Responsible for working with colleagues to improve the quality of services provided to clients/patients.
  • Participate in annual audits relevant to the clinical pathway.
  • Participate in the First Episode Psychosis Duty Rota.
Requirements
  • Appropriate professional registration.
  • Understanding of First Episode Psychosis/wait time standards.
  • Experience of Family Work (desirable).
