Health Inequalities Outreach Coordinator

We are seeking a highly motivated and experienced Health Inequalities Outreach Coordinator to join our team at West View Surgery. The successful candidate will play a key role in supporting patients from diverse backgrounds who are affected by health inequalities.
Key Responsibilities- Coordinate and deliver activities that promote early diagnosis of cancer and support patients in accessing healthcare services.
- Develop and implement targeted interventions to engage hard-to-reach communities and promote cancer screening.
- Work closely with the Cancer Screening Programmes teams to identify and address barriers to timely presentation.
- Support the development of new interventions and information resources to target high-risk communities.
- Collaborate with local pharmacies to promote smoking cessation and reduce the smoking population in South Tyneside.
- Work with Social Prescribing Link Workers to reduce obesity and promote healthy lifestyles.
- Educated to GCSE level or equivalent.
- GCSE Mathematics & English (C or above).
- Experience of working with the general public, administrative duties, and healthcare settings.
- Excellent communication skills, strong IT skills, and ability to work autonomously.
- Competent in the use of Office and Outlook, effective time management, and problem-solving skills.
