We are seeking an experienced professional to join our team as a Support Coordinator for our South Tyneside Learning Disability Service.
Your Role
As a Support Coordinator, you will be responsible for providing high-quality support and care to individuals with learning disabilities, promoting their independence and well-being.
Key responsibilities include:
- Support and Care Provision
- Providing practical and emotional support to individuals with learning disabilities
- Developing and implementing personalized care plans to meet the unique needs of each individual
- Service Coordination
- Collaborating with individuals, families, and healthcare professionals to ensure seamless care and support
- Coordinating and managing care packages to meet the needs of individuals
- Staff Supervision and Development
- Supervising and supporting staff to ensure they have the necessary skills and knowledge to provide high-quality care
- Developing and implementing training programs to enhance staff skills and knowledge
Requirements
To be successful in this role, you will need:
- Experience working with individuals with learning disabilities
- Strong communication and interpersonal skills
- Ability to work flexibly and adapt to changing circumstances
- Valid driving license and access to a vehicle (desirable)
