Support Coordinator Role

We are seeking a skilled and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in supporting our customers who have complex needs to achieve their goals and improve their overall well-being.

Key Responsibilities:

  • Work closely with customers, their families, and other stakeholders to understand their needs and develop personalized support plans.
  • Provide emotional support, guidance, and advocacy to customers to help them overcome challenges and achieve their goals.
  • Collaborate with other professionals, such as healthcare providers and social workers, to ensure a comprehensive and coordinated approach to customer support.
  • Monitor and evaluate the effectiveness of support plans and make adjustments as needed.

Requirements:

  • Professional experience and qualifications in a relevant field, such as social work or healthcare.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and stakeholders.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to work.
  • Knowledge of relevant legislation and policies, such as the Care Act and the Mental Health Act.
