About the Role

We are seeking a skilled and compassionate Community Health Assistant to join our team at Derbyshire Community Health Services NHSFT. As a Community Health Assistant, you will play a vital role in providing high-quality care and support to patients in their homes.

Key Responsibilities
  • Provide personal care and support to patients, promoting their independence and well-being.
  • Work collaboratively with healthcare professionals to deliver effective care plans and programs.
  • Assist with administrative tasks, such as documenting patient information and maintaining accurate records.
  • Travel to patients' homes, using your own vehicle, to provide care and support.
Requirements
  • GCSE Maths and English (C/4 or above) or equivalent.
  • Previous experience in a health and social care setting (minimum 1 year).
  • Excellent communication and interpersonal skills.
  • Ability to work in a team and willingness to travel within a locality.
