About the Role

We are seeking a highly motivated and compassionate Community Health Assistant to join our team at Derbyshire Community Health Services NHS Foundation Trust. As a Community Health Assistant, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Provide personal care and support to patients, including assistance with daily living activities and mobility.
  • Work collaboratively with healthcare professionals to develop and implement individual care plans.
  • Conduct regular assessments and reviews to ensure patient needs are met and care plans are updated accordingly.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • GCSE Maths and English (C/4 or above) or equivalent.
  • Educated to NVQ 3 level (or equivalent) in a relevant subject or equivalent previous proven experience.
  • IT skills and ability to use electronic patient records.
  • Previous experience in a health and social care setting (minimum 1 year).
  • Experience providing personal care and support to individuals with varying needs.
