About the Role

We are seeking a skilled and compassionate Community Health Assistant to join our team at Derbyshire Community Health Services NHSFT. As a Community Health Assistant, you will play a vital role in providing high-quality care and support to patients in their own homes.

Key Responsibilities

  • Provide personal care and support to patients, including assistance with daily living activities and medication management.
  • Work collaboratively with healthcare professionals to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure patients receive the best possible care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements

  • GCSE Maths and English (C/4 or above) or equivalent.
  • Education to NVQ 3 level (or equivalent) in a relevant subject or equivalent previous proven experience.
  • IT skills.
  • Previous experience in a health and social care setting (minimum 1 year).
  • Experience providing personal care.
  • Understanding of the health and social care system.
  • Excellent communication and interpersonal skills.
  • Ability to work in a team.
  • Willingness to travel within a locality.
