About the Role

We are seeking a highly motivated and enthusiastic Phlebotomist to join our Integrated Community Teams across Derby City. As a Community Health Specialist, you will play a vital role in providing excellent patient-centred care to people in their own homes and community healthcare settings.

Key Responsibilities

  • Carry out venepuncture and other appropriate healthcare procedures in patients' own homes or in community healthcare settings, acting within the procedures outlined in the Phlebotomy competency framework.
  • Undertake appropriate patient care assessments as required, such as baseline observations.
  • Escalate any concerns regarding patients' welfare to the registered members of the Integrated Community Team in a timely manner.
  • Enter contemporaneous notes in the relevant patient information system.
  • Use DCHS patient information systems as needed, including accepting electronic referrals.
  • Recognise the importance of confidentiality and sensitivity of issues at all times, and be able to act using the utmost discretion and confidentiality.

Person Specification

  • Essential:
    • GCSE level at Grade C / Level 4 or above (English) or equivalent
    • Willing to undertake the Care Certificate
    • Experience of team working and versatility to take on a variety of tasks
    • Knowledge of confidentiality issues and Caldicott principles
    • Awareness of Health and Social Care sector
  • Desirable:
    • NVQ 2 Health/Social Care
    • Venepuncture trained
