Dementia Palliative Care Assistant

We are seeking a highly motivated and compassionate Dementia Palliative Care Assistant to join our team at Derbyshire Community Health Services NHSFT. As a key member of our Dementia Palliative Care Team, you will play a vital role in delivering high-quality care to individuals living with dementia and palliative care needs.
Key Responsibilities- Support people with dementia and palliative care needs, and their carers, across Derbyshire.
- Work as an integral part of a small team to deliver palliative and end-of-life care in an individualized and person-centered way.
- Support the delivery of palliative care in people's own homes and care homes.
- Collaborate with registered clinicians to ensure seamless care delivery.
- Support discharges from acute hospital beds and pathway 3.
Derbyshire Community Health Services NHSFT is a leading provider of community health services, caring for over one million people in Derbyshire and Derby. We employ around 4,200 staff, working in various settings, including community hospitals, health centers, clinics, and care homes.
Requirements- NVQ level 3 in care or equivalent.
- Experience of working in a healthcare setting.
