Position Overview

We are seeking a dedicated Palliative Care Physiotherapist to join our Independent Living Team. Our mission is to enhance the quality of life for our beneficiaries, and this role is crucial in advancing our commitment to exceptional end-of-life care. Collaborating with our Occupational Therapist and Therapy Assistant, you will contribute to a model of rehabilitative care that positively impacts the lives of those we serve, both within our facility and in their residences.

Key Responsibilities

In this role, you will provide support to patients, their families, and caregivers who are facing complex physical, emotional, and spiritual challenges associated with their palliative conditions. You will work closely with a multi-disciplinary team, including our Alliance partners, to ensure that specialized therapy resources are utilized effectively. This will involve both individual and group interventions aimed at delivering community-based services.

Job Duties

  • Conduct comprehensive physiotherapy assessments and rehabilitation for patients with diverse and complex needs, utilizing advanced clinical reasoning to identify therapeutic and clinical requirements.
  • Manage a clinical caseload of palliative patients with varying complexities, prioritizing, reviewing, and discharging as necessary.
  • Serve as a resource for specialized physiotherapy knowledge, offering guidance and support to hospice staff, volunteers, and the broader community.
  • Develop and facilitate empowering, patient-centered self-management therapeutic groups, collecting outcome measures and feedback as appropriate, and adjusting group objectives as needed.
  • Influence palliative rehabilitation practices across various care settings as part of the broader patient and caregiver support team, promoting the model of Rehabilitative Palliative Care within the hospice and the community.
  • Contribute to the service model that addresses the rehabilitation needs of patients and caregivers as they navigate dying, death, and grief.
  • Actively embody and promote the values of St Nicholas Hospice Care: Compassion, Accountability, Respect, and Equity.

  • Willingness to work flexibly to accommodate patient needs
  • Proficiency in various IT systems and software, including Word, Excel, PowerPoint, Office 365, and electronic patient record systems (such as SystmOne), as well as virtual platforms like Zoom and MS Teams

Required Qualifications

Essential Qualifications

  • Degree or Diploma in Physiotherapy
  • HPC Registration
  • Evidence of Continuing Professional Development (CPD)
  • Strong understanding of palliative care theory and practice

Desirable Qualifications

  • Advanced Communications Skills Training
  • Leadership training or qualification
  • Evidence of Master's degree or equivalent level study
  • Additional qualifications in therapeutic interventions such as acupuncture or core counseling

Experience Requirements
