Rennie Grove Peace Hospice Care is seeking a highly skilled and compassionate Palliative Care Social Worker to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing holistic care and support to patients and their families.
Key Responsibilities- Assess and develop care plans that address the psychosocial, welfare rights, and practical needs of patients and their families.
- Contribute to the implementation and evaluation of service developments, ensuring they align with the hospice's strategic objectives and vision.
- Line manage Assistant Social Workers, providing regular supervision and support.
- Attend and facilitate Protected Learning sessions to enhance the clinical skills of patient services staff.
- Maintain a social worker peer support network across providers.
- Undertake service audits, including Safeguarding and Carers audits, and encourage research and individual service evaluation.
- Ensure operational policies, guidelines, and risk assessments are in place to support new service developments.
We are looking for a candidate with:
- A minimum of a degree or Dip Q or CQSQ or equivalent.
- Registration with the HCPC.
- Experience of working in a related care setting.
- Experience of working as a social worker for at least 2 years.
- Experience of working and communicating with people pre and post bereavement.
- Experience of leading service developments and projects.
