We are seeking an experienced and skilled Palliative Care Team Lead to join our Community Team at St Nicholas Hospice Care. As a key member of our multi-disciplinary team, you will be responsible for leading the community hospice nursing team and influencing the holistic care of patients and carers with end of life and specialist palliative care needs.
Key Responsibilities- Provide specialist palliative care advice and support to patients in West Suffolk and Thetford.
- Lead the community hospice nursing team and provide clinical and day-to-day management to Clinical Nurse Specialists, Registered Nurses, and Healthcare Assistants.
- Support the ongoing development of the palliative care community responsive team.
- Work with the wider multi-disciplinary team to provide face-to-face and telephone support for a small caseload of patients.
- Participate in answering calls from our telephone advice line.
- Adult Registered Nurse (active NMC registrant)
- Relevant diploma or degree in Nursing
- Level 7 Master's level study (e.g. Advanced Communication Skills, Clinical Assessment)
- Evidence of continued professional development
- Completion of Master's degree
- Non-medical prescribing qualification
- Understanding of the value of multi-disciplinary team working
- Significant experience of working autonomously
- Robust understanding of own professional and clinical boundaries
- Awareness of own emotional responses and ability to respond to these appropriately
- Ability to reflect and critically appraise own performance
- Ability to motivate self and others and to lead by example
- Able to recognise and consider professional and ethical issues, seeking support as appropriate
- Driving license, use of own car, business insurance
St Nicholas Hospice Care is a local charity providing help, advice, and support to people in West Suffolk and Thetford who are living with long-term and life-threatening illnesses. Our services are not just for patients, but for their family and friends too. They include everything from specialist medical care to bereavement support and practical help.
