We’re looking for a Social Worker – Palliative Care who will be responsible for the Social Worker Assistant team in the outpatient unit, overseeing discharge planning and the provision of practical, social and emotional support services for patients and their families
We’re wanting to hear from people who have experience undertaking holistic physiotherapy assessments and treatment of patients with complex, life limiting illness as an autonomous practitioner with support from the Senior Physiotherapist.
You'll maintain a respectful non-judgmental and caring attitude ensuring the Rennie Grove Peace Hospice vision and philosophy of care is integral to the care delivered and undertaken. You’ll also act as a Rennie Grove Peace ambassador within the community.
Further to this you’ll quickly build trusting, empathetic and caring relationships with volunteers, customers, employees and head office teams, not forgetting local traders and the community
What you’ll bring
Minimum of a degree or Dip Q or CQSQ or equivalent
Must be registered with the HCPC
Experience of working in a related care setting
Experience of working as a social worker for at least 2 years
Understanding of palliative care and Hospice philosophy
Understanding of the ethnic, cultural and religious influences on patients and their families and how these affect (or are affected) by life threatening illness or bereavement.
Full, current valid UK driving licence
The following would be advantageous but are not essential:
Qualification in the field of exercise / palliative care / physiotherapy
Experience of being part of a multi professional team
Experience working within palliative care
