Position Overview

The Palliative Care Social Services Coordinator will lead the Social Worker Assistant team within the Inpatient Unit, ensuring effective discharge planning and delivering essential practical, social, and emotional support services to patients and their families. This role will serve as a vital resource for all members of the multidisciplinary team (MDT) regarding the psychosocial dimensions of patient and family care, offering social work support and guidance to community teams and outpatient services.

Key Responsibilities

  • Oversee the provision of practical and psychosocial assistance to inpatients and their families, facilitating the discharge planning process in collaboration with statutory care providers.
  • Provide social work support and counsel to community teams and outpatient services, including the establishment of welfare clinics.
  • Manage the Social Worker Assistant, mentor and supervise students, and coordinate a team of wellbeing volunteers to ensure consistent support for patients, including spiritual care.
  • Conduct monthly safeguarding training for clinical staff as part of the clinical induction program, covering topics such as Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and chaperone policies.
  • Participate in the Patient Safety Group, focusing on safeguarding initiatives.
  • Lead specific project work that aligns with agreed service enhancements, including the development and maintenance of the carers' pathway, while collaborating closely with the Supportive Care team.
  • Coordinate care packages tailored to the needs of patients and their families.
  • Enhance the social work services at Rennie Grove Peace by sharing national best practices throughout the organization.

Qualifications and Experience

  • Essential: A minimum of a degree or equivalent qualification in social work, with registration from the Health and Care Professions Council (HCPC).
  • Desirable: Additional qualifications in exercise, palliative care, or physiotherapy.
  • Essential: At least two years of experience as a social worker in a relevant care setting, with a proven track record of working with individuals pre and post-bereavement.
  • Desirable: Experience in leading service developments and projects, as well as working within a multi-professional team.

Skills and Attributes

  • Strong understanding of palliative care and hospice philosophy.
  • Proficient in Microsoft Office applications, including Word, Excel, and Outlook.
  • Able to build effective working relationships and communicate clearly, both verbally and in writing.
  • Excellent organizational skills with attention to detail.
  • Commitment to ongoing professional development and regular supervision.
  • Empathetic and sensitive to the needs of volunteers and clients, many of whom may have experienced bereavement.
