Position Overview
An exceptional opportunity has emerged to become a pivotal member of our Community Team as the Lead Community Palliative Care Coordinator. This team is an integral part of our well-established multi-disciplinary group, which includes clinical nurse specialists, senior hospice nurses, and healthcare assistants dedicated to offering specialized support to patients and families facing life-limiting illnesses within our community.
The selected candidate will oversee the community hospice nursing team, shaping the comprehensive care provided to patients and caregivers with end-of-life and specialized palliative care requirements. This role presents a unique chance to significantly impact the lives of individuals and their families.
Key Responsibilities
You will engage with the Community Team to deliver expert palliative care guidance and assistance to patients in the designated areas. Your role will encompass both clinical oversight and daily management of Clinical Nurse Specialists, Registered Nurses, and Healthcare Assistants within the Community Team. You will also be expected to foster the ongoing development of the responsive palliative care community team. Additionally, you will collaborate with the broader multi-disciplinary team, providing both in-person and telephonic support for a select group of patients, and participate in managing inquiries from our advice line.
The ideal candidate will exhibit a high degree of clinical and managerial expertise, possess outstanding communication skills, and demonstrate the ability to adapt to the evolving needs of the service.

Qualifications and Skills
Essential Qualifications:
- Active Adult Registered Nurse (NMC registrant)
- Relevant diploma or degree in Nursing
- Master's level study (e.g., Advanced Communication Skills, Clinical Assessment)
- Evidence of ongoing professional development
Desirable Qualifications:
- Completion of a Master's degree
- Non-medical prescribing qualification

Experience Requirements:
Essential Experience:
- Extensive post-registration experience in palliative care or oncology
- In-depth knowledge and experience in managing a palliative care caseload
