Key Responsibilities
The successful candidate will manage a diverse caseload and address a wide array of health concerns in a primary care environment, ensuring exceptional care standards for all patients.
Collaborative Relationships
You will work closely with all practice personnel, including PCN ARRS staff, district nurses, health visitors, midwives, and retinal screening teams.
Clinical Duties
In line with the practice schedule, the post-holder will be available to perform various tasks, including conducting patient consultations, handling telephone inquiries, visiting patients at home, managing repeat prescriptions, reviewing lab results, and addressing correspondence promptly.
Home Visits
The role requires the execution of home visits as outlined in the home visit protocol, typically following morning consultations.
On-Call Responsibilities
Participation in on-call duties as part of a rotation is expected.
About Plowright Medical Centre
At Plowright Medical Centre, you will provide care for a stable patient population that values general practice in a semi-rural setting. Our practice is known for its supportive and friendly atmosphere, combined with a high level of organization and efficiency, allowing GPs to focus on their clinical expertise.
We are part of a collaborative network, with access to a diverse healthcare team, including health coaches, mental health practitioners, physiotherapists, social prescribers, pharmacists, and in-house counselors.
Utilizing the SystmONE system, we have a full complement of administrative staff to alleviate GP administrative burdens, supported by a robust IT and administrative framework.
Our practice consistently achieves high QOF scores and receives positive feedback from our patients.
Responsibilities Overview
Primary Duties
The role encompasses a variety of clinical responsibilities, including but not limited to:
- Conducting consultations and managing patient care
- Making informed, autonomous decisions regarding patient health
- Assessing healthcare needs for undiagnosed conditions
- Screening for disease risk factors
- Developing care plans in collaboration with patients
- Providing health education and counseling
- Managing patient admissions and referrals
- Maintaining accurate and timely patient records
- Prescribing medications as appropriate
Home Visits
Home visits will be conducted as per the established protocol, usually after morning consultations.
Additional Responsibilities
The post-holder will adhere to all relevant practice policies and guidelines, maintain confidentiality, and engage in continuous professional development.
Quality Assurance
The post-holder will strive to uphold quality standards within the practice, alerting team members to any quality or risk issues and contributing to the overall effectiveness of the team.
