About the Role

The Campingland Surgery is seeking a qualified and seasoned pharmaceutical care specialist to join our dynamic team on a part-time basis, contributing 19 hours per week across two days. This role is integral to our multi-disciplinary approach to patient care.

Main Responsibilities:

  • Conducting clinical assessments and providing treatment recommendations based on extensive knowledge of medications for various health conditions.
  • Offering specialized guidance on the effective use of pharmaceuticals.
  • Performing comprehensive medication reviews and delivering health education to patients through in-person consultations, phone calls, or written communications.
  • Leading initiatives focused on patient-centered medication optimization and quality enhancement.
  • Assisting with the practice's dispensary operations.
  • Serving as the primary contact for all inquiries related to medications.
  • Facilitating improvements in clinical practices through audits and the implementation of necessary changes.
  • Engaging in care planning, including mental health considerations, strategies to prevent hospital admissions, and support for end-of-life care.
  • Managing minor ailment assessments and directing patients according to their clinical needs.
  • Documenting essential patient health information during consultations and contributing to quality outcome frameworks.
  • Providing expert clinical advice regarding medication management.
  • Building and maintaining professional relationships with patients.
  • Coordinating with other healthcare providers, including secondary care and community pharmacies, to ensure seamless transitions of care.
  • Participating in a professional clinical network and accessing appropriate clinical supervision.
  • Overseeing the repeat prescription authorization process by evaluating requests and monitoring medication review timelines.

About the Candidate

The ideal candidate will possess a pharmacy degree and have substantial experience in primary care, demonstrating the following qualifications:

  • Registered with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy settings, such as hospitals, primary care, or community environments.
  • Qualified as an independent prescriber or actively pursuing this qualification.
  • Membership with the Royal Pharmaceutical Society (RPS).
  • Familiarity with common acute and chronic conditions encountered in general medical practice.
  • Proven pharmaceutical skills and knowledge in essential areas.
  • Accountability for delivering professional expertise and direct service provision.
  • Ability to prioritize effectively when addressing problems and to refer to senior staff or GPs as necessary.
  • Adherence to legal, ethical, and professional standards and policies.
  • Support patients in making informed decisions regarding their prescribed medications in line with NICE guidelines.
  • A flexible, non-judgmental approach with adaptability to various situations.
  • Self-motivated with strong communication abilities.
  • Commitment to maintaining patient confidentiality.

About Us

The Campingland Surgery is a prominent GP practice dedicated to providing high-quality healthcare to a patient population of approximately 7,500. Established in 1965, our practice is known for its family-oriented and traditional healthcare approach, supported by a team of five GPs. We operate an onsite dispensary and offer medication support to local care homes. Our current CQC rating reflects our commitment to excellence in patient care.

Disclosure and Barring Service Check
