About the Role

The Campingland Surgery is seeking a qualified and experienced Pharmaceutical Care Specialist to join our dynamic team on a part-time basis, contributing 19 hours per week across two days. This role is integral to our multi-disciplinary approach to patient care.

Main Responsibilities:

  • Conducting clinical assessments and providing treatment for patients, utilizing extensive knowledge of medications for various health conditions.
  • Offering specialized guidance on medication management.
  • Performing comprehensive medication reviews and delivering health education to patients through in-person consultations, phone calls, or written communications.
  • Leading initiatives focused on patient-centered medication optimization and quality enhancement.
  • Assisting in the practice dispensary operations.
  • Serving as a primary resource for all inquiries related to medications.
  • Facilitating improvements in clinical practices through audits and the implementation of necessary changes.
  • Engaging in care planning, which includes mental health considerations, strategies to prevent hospital admissions, and end-of-life care.
  • Managing minor ailment assessments and directing patients based on their clinical needs.
  • Documenting relevant patient health information during consultations and contributing to quality outcome frameworks.
  • Providing expert clinical advice regarding medications.
  • Building and maintaining professional relationships with patients.
  • Coordinating with other healthcare providers, including secondary care and community pharmacies, to ensure seamless patient care transitions.
  • Participating in a professional clinical network and accessing appropriate clinical supervision.
  • Overseeing the repeat prescription authorization process by evaluating requests and managing medication review timelines.

About the Candidate

The ideal candidate will possess a pharmacy degree and have substantial experience in primary care, along with the following qualifications:

  • Registered with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy settings (hospital, primary care, or community) or actively pursuing this experience.
  • Qualified as an independent prescriber or in the process of obtaining this qualification.
  • Membership with the Royal Pharmaceutical Society (RPS).
  • Understanding of common acute and chronic conditions encountered in general medical practice.
  • Demonstrated pharmaceutical skills and knowledge in essential areas.
  • Accountability for delivering professional expertise and direct service as an individual.
  • Ability to prioritize effectively when addressing problems and to refer to senior colleagues or GPs confidently when necessary.
  • Adherence to legal, ethical, professional, and organizational policies and codes of conduct.
  • Capability to assist patients in making informed decisions regarding prescribed medications and promoting adherence in line with NICE guidelines.
  • A non-judgmental and adaptable approach.
  • Self-motivated with exceptional communication skills.
  • Commitment to maintaining patient confidentiality.

About Us

The Campingland Surgery is a prominent GP practice, established in 1965, dedicated to serving a patient population of approximately 7,500. We pride ourselves on a family-oriented, traditional approach to healthcare, supported by a partnership of five GPs. Our facility includes an onsite dispensary, providing medication support to local care homes. Our current CQC rating reflects our commitment to quality care.

Disclosure and Barring Service Check

This position is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, necessitating a Disclosure submission to the Disclosure and Barring Service to check for any prior criminal convictions.
