About the Role
The Campingland Surgery is seeking a qualified and experienced Pharmaceutical Care Specialist to join our dynamic team on a part-time basis, working 19 hours per week across two days. This role is integral to our multi-disciplinary approach to patient care.
Main Responsibilities:
- Conduct clinical assessments and provide treatment recommendations, utilizing in-depth knowledge of pharmaceuticals for various health conditions.
- Offer specialized guidance on medication usage.
- Perform comprehensive medication reviews and deliver health education to patients through in-person consultations, phone calls, or written communications.
- Lead initiatives focused on patient-centered medication optimization and quality enhancement.
- Assist in the practice dispensary operations.
- Serve as the primary contact for all inquiries related to medications.
- Facilitate improvements in clinical care by conducting practice-based audits and implementing necessary changes.
- Engage in care planning for diverse patient needs, including mental health, admission avoidance strategies, and end-of-life care.
- Manage minor ailment assessments and direct patients based on their clinical requirements.
- Document relevant patient health information during consultations, contributing to quality outcome measures.
- Provide expert clinical advice regarding medications.
- Establish and maintain professional relationships with patients.
- Collaborate with other healthcare providers, including secondary care and community pharmacies, to ensure safe transitions of care.
- Participate in a professional clinical network and access appropriate clinical supervision.
- Oversee the repeat prescription reauthorization process by evaluating requests for repeat medications and monitoring review timelines.
About the Candidate
The ideal candidate will possess the following qualifications and skills:
- A degree in pharmacy and registration with the General Pharmaceutical Council.
- Post-qualification experience in pharmacy, whether in hospital, primary care, or community settings.
- Qualification as an independent prescriber or actively pursuing this credential.
- Membership in the Royal Pharmaceutical Society (RPS).
- Familiarity with common acute and chronic conditions typically encountered in a general medical practice.
- Demonstrated proficiency in core pharmaceutical skills and knowledge.
- Accountability for delivering professional expertise and direct service provision.
- Ability to prioritize effectively when addressing problems and to refer to senior colleagues or GPs when necessary.
- Adherence to legal, ethical, professional, and organizational standards and codes of conduct.
- Support patients in making informed decisions regarding prescribed medications, in line with NICE guidelines.
- A non-judgmental and adaptable approach.
- Self-motivated with exceptional communication abilities.
- Commitment to maintaining patient confidentiality.
About Us
The Campingland Surgery is a prominent GP practice, established in 1965, dedicated to providing quality healthcare to a patient population of approximately 7,500. We pride ourselves on a family-oriented, traditional approach to medical care, supported by a partnership of five GPs. Our practice includes an onsite dispensary and offers medication support to local care homes. We are proud to maintain a good rating from the Care Quality Commission (CQC).
