About the Role

The Campingland Surgery is seeking a qualified and experienced Pharmaceutical Care Specialist to join our dynamic practice on a part-time basis, contributing 19 hours per week across two days. This role is integral to our multi-disciplinary team, focusing on delivering exceptional patient care.

Main Responsibilities:

  • Conducting clinical assessments and treatments for patients, utilizing expert knowledge of pharmaceuticals for various health conditions.
  • Offering specialized guidance on medication use.
  • Performing comprehensive medication reviews and providing health education to patients through in-person consultations, phone calls, or written communications.
  • Leading initiatives on patient-centered medication optimization and quality enhancement.
  • Assisting in the practice's dispensary operations.
  • Serving as a primary contact for all inquiries related to medications.
  • Facilitating improvements in clinical practices through audits and implementing necessary changes.
  • Engaging in care planning, including mental health strategies, admission avoidance, and end-of-life care.
  • Managing minor ailment assessments and directing patients based on their clinical needs.
  • Documenting relevant patient health information during consultations and contributing to quality outcome frameworks.
  • Providing expert clinical advice regarding medications.
  • Building and maintaining professional relationships with patients.
  • Coordinating with other healthcare providers, including secondary care and community pharmacies, to ensure safe transitions of care.
  • Participating in a professional clinical network and accessing appropriate clinical supervision.
  • Overseeing the repeat prescription authorization process by reviewing requests and monitoring medication review dates.

About the Candidate

The ideal candidate will possess a pharmacy degree and be registered with the General Pharmaceutical Council, along with:

  • Post-qualification experience in pharmacy, whether in hospital, primary care, or community settings.
  • Independent prescriber qualification or actively pursuing this credential.
  • Membership in the Royal Pharmaceutical Society (RPS).
  • Understanding of common acute and chronic conditions encountered in general medical practice.
  • Demonstrated pharmaceutical skills and knowledge in essential areas.
  • Accountability for delivering professional expertise and direct service as an individual.
  • Ability to prioritize effectively and identify deviations from normal patterns, referring to senior staff or GPs when necessary.
  • Commitment to adhering to legal, ethical, professional, and organizational policies and procedures.
  • Support for patients in making informed decisions about their prescribed medications in line with NICE guidelines.
  • A non-judgmental approach with flexibility and adaptability.
  • Self-motivation and excellent communication skills.
  • Understanding of the importance of patient confidentiality.

About Us

The Campingland Surgery is a prominent GP practice, dedicated to providing high-quality healthcare to a patient population of approximately 7,500. Established in 1965, we pride ourselves on a family-oriented, traditional approach to healthcare, supported by a partnership of five GPs. Our practice includes an onsite dispensary and offers medication support to local care homes. We are committed to maintaining a good standard of care as reflected in our current CQC rating.
