About the Role

The Campingland Surgery is seeking a qualified and experienced Pharmaceutical Care Specialist to join our dynamic team on a part-time basis, contributing 19 hours per week across two days. This role is integral to our multi-disciplinary team, focusing on enhancing patient care through expert pharmaceutical knowledge.

Main Responsibilities:

  • Conducting clinical assessments and treatments for patients, utilizing in-depth knowledge of medications for various health conditions.
  • Offering specialized guidance on medication use.
  • Delivering comprehensive medication reviews and health education to patients through face-to-face consultations, phone calls, or written communications.
  • Leading initiatives on patient-centered medication optimization and quality enhancement.
  • Assisting with the practice's dispensary operations.
  • Serving as a primary resource for all medication-related inquiries.
  • Facilitating improvements in clinical care via practice-based audits and implementing necessary changes.
  • Engaging in care planning, including mental health considerations, admission avoidance strategies, and end-of-life care.
  • Managing minor ailment triage and directing patients based on clinical needs.
  • Documenting relevant patient health information during consultations and contributing to quality outcome frameworks.
  • Providing clinical advice and expertise regarding medications.
  • Building and maintaining professional relationships with patients.
  • Coordinating with other healthcare providers, such as secondary care and community pharmacies, to ensure safe transitions of care.
  • Participating in a professional clinical network and accessing appropriate clinical supervision.
  • Overseeing the repeat prescription reauthorization process by reviewing requests and managing medication review timelines.

About the Candidate

The ideal candidate will possess the following qualifications and skills:

  • A completed degree in pharmacy and registration with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy, whether in hospital, primary care, or community settings.
  • Qualification as an independent prescriber or actively working towards this credential.
  • Membership in the Royal Pharmaceutical Society (RPS).
  • Familiarity with common acute and chronic conditions typically encountered in a general medical practice.
  • Demonstrated pharmaceutical skills and knowledge in core areas.
  • Accountability for delivering professional expertise and direct service provision.
  • Ability to prioritize effectively when problem-solving and to refer to senior staff or GPs when necessary.
  • Adherence to legal, ethical, professional, and organizational policies and procedures.
  • Capability to assist patients in making informed decisions about their prescribed medications and promoting adherence according to NICE guidelines.
  • A non-judgmental attitude with flexibility and adaptability.
  • Self-motivated with strong communication skills.
  • Understanding of the importance of patient confidentiality.

About Us

The Campingland Surgery is a prominent GP practice, established in 1965, dedicated to providing exceptional healthcare to a patient population of approximately 7,500. Our practice is known for its family-oriented, traditional approach to healthcare, supported by a team of five GPs. We operate an onsite dispensary and offer medication support to local care homes. Our current CQC rating reflects our commitment to quality care.
