The service focuses on two key areas:

Active outreach support for individuals facing street homelessness A tailored 6-bedroom accommodation service for those with a history of homelessness and complex needs, which may encompass mental health, substance misuse, and physical health challenges.

Key Responsibilities

As a complex care navigator, your role will be diverse and adaptable. Initially, you will facilitate access to services by engaging with rough sleepers at street level and initiating a street-based assessment to determine the most suitable accommodation support needed. You must possess the confidence and experience to earn the trust of rough sleepers and the ability to advocate for them with various housing providers.

Following the assessment, the complex care navigator will continue to collaborate with guests and providers across housing, mental health, and substance misuse to support their transition into and sustainability of accommodation. Your role will involve providing motivational support to guests in dispersed housing, including Turning Point's new supported accommodation, to help them maintain engagement with health professionals and encourage adherence to their ongoing treatment and recovery journeys.

As a complex needs navigator, you will work closely with other providers and colleagues to gather and coordinate essential information about guests with multi-agency providers in mental health and substance misuse, acting swiftly to advocate for or coordinate rapid multi-agency interventions to avert impulsive actions that could lead to harmful situations and jeopardize housing stability.

Ideal Candidate Profile

You will have relevant lived experience through direct engagement with health and social care services. Our ideal candidate will hold a Level 3 award in Understanding Substance Misuse, along with training in mental health and trauma-informed practices. Alternatively, you should demonstrate a substantial history or knowledge of working with homelessness and rough sleeping.

You should be driven to empower individuals who often lack a platform for their voices to be heard, showcasing passion, energy, enthusiasm, and empathy. Being resourceful and innovative in your approach to working with individuals facing homelessness is essential, as is the ability to work confidently and resiliently while leading aspects of service development. Additionally, experience in coordinating multi-agency approaches and mobilizing rapid interventions to prevent breakdowns in support is crucial.

Strong written and verbal communication skills, along with proficiency in IT and digital systems, are also important.
