The position focuses on fostering a dynamic community rather than merely providing a service, emphasizing shared decision-making.
The service operates with two primary objectives:
Engaging in proactive outreach to support individuals experiencing street homelessness. Offering a tailored accommodation service with six rooms for those with a history of homelessness and complex needs, which may encompass mental health, substance misuse, and physical health challenges.At Turning Point, we are dedicated to supporting individuals across the UK with mental health concerns. Your role will be instrumental in positively impacting their lives as you collaborate closely with them to help them realize their potential. Turning Point boasts several well-established services in Staffordshire, and you will be part of a broader supportive network in the region, with opportunities for future advancement.
Key Responsibilities
As a complex needs care navigator, your responsibilities will be diverse and adaptable. Initially, you will facilitate access to services by engaging with individuals experiencing homelessness at street level and conducting a street-based assessment to determine the most suitable accommodation support needed. Building trust and rapport with rough sleepers will be essential, as will advocating for them with various housing providers.
Following the assessment, you will continue to collaborate with guests and service providers across housing, mental health, and substance misuse sectors to ensure a smooth transition into accommodation and its sustainability. Your role will involve providing motivational support to guests in dispersed housing, including Turning Point's new supported accommodation, encouraging them to engage with health professionals and maintain their treatment and recovery journeys.
As a complex needs navigator, you will work in close partnership with other service providers to gather and coordinate essential information about guests, facilitating rapid interventions to prevent crises that could jeopardize their housing stability.
Ideal Candidate Profile
The ideal candidate will possess relevant lived experience through direct engagement with health and social care services. A Level 3 qualification in Understanding Substance Misuse, along with training in mental health and trauma-informed practices, is preferred. Alternatively, substantial experience or knowledge in working with homelessness and rough sleeping will be considered.
You should be driven to empower individuals who often lack a voice, demonstrating passion, energy, enthusiasm, and empathy. Resourcefulness and innovation in your approach to working with those experiencing homelessness are crucial, as is the ability to work confidently and resiliently in service development. Experience in coordinating multi-agency efforts and mobilizing rapid interventions to prevent service breakdown is also essential.
Strong written and verbal communication skills, along with proficiency in IT and digital systems, are important for this role.
