About the Role

We are seeking a highly skilled and compassionate Mental Health Nurse to join our team at Lincolnshire Partnership NHS Foundation Trust. As a Band 5 Nurse, you will play a vital role in providing high-quality care to older adults experiencing mental health problems associated with dementia.

Main Responsibilities
  • Deliver person-centered care to patients with complex needs, working closely with the multidisciplinary team to develop and implement care plans.
  • Conduct comprehensive assessments and risk evaluations to identify individualized care needs and develop strategies to promote patient autonomy and independence.
  • Collaborate with patients, carers, and healthcare professionals to ensure seamless transitions and effective discharge planning.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
  • Supervise and mentor junior staff, providing guidance and support to ensure high-quality care delivery.
Requirements
  • Registered Mental Health Nurse (RMN) or equivalent qualification.
  • Experience working with older adults with dementia and complex mental health needs.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • Ability to travel independently across the county and work a range of shifts, including evenings, nights, and weekends.
