Position Overview

An exceptional opportunity has emerged for a dedicated and passionate pharmacist possessing outstanding clinical and interpersonal abilities to spearhead and provide a superior service to the Integrated Care Operations (ICO) division at University College London Hospitals (UCLH).

The ICO manages a variety of services aimed at ensuring safe and effective transitions of care. These services encompass:

- A home nursing initiative designed to facilitate safe discharges for patients under the care of a UCLH Consultant.

- The Integrated Discharge Service (IDS), which collaborates closely with North Central London (NCL) Integrated Care Services to offer an Admission Avoidance program for the Emergency Department through pathways such as Camden and Islington Virtual Wards, along with tailored services including the Camden Adult Pathway partnership (CAPP), a Homeless Intermediate Care initiative, and a Rough Sleeper Nurse collaborating with Camden Street teams.

The successful candidate will integrate into the Medicine and Emergency Services pharmacy team to deliver pharmaceutical services and ensure safe transitions of care between the ICO and other NCL Integrated Discharge Services.

Key Responsibilities

Responsibilities will encompass coordinating and engaging in the daily clinical pharmacy service for the ICO services team, as well as participating in relevant meetings and handovers. You will collaborate with the ICO services team on initiatives aimed at developing, implementing, maintaining, and enhancing new services.

Utilizing your independent prescribing expertise, you will optimize medication regimens in preparation for patient discharges and serve as a resource for staff and patients regarding medication inquiries.

As a member of the multidisciplinary team (MDT), you will contribute to audits, service enhancements, and quality improvement projects related to existing systems and processes.

Additional leadership and management duties for the ICO and ESD will include governance, analysis and reporting of medication expenditures, conducting audits, and evaluating services, along with providing education and training to the MDT.

Within the pharmacy department, you will engage in local and national pharmacy initiatives and support the execution of educational and developmental programs.
