Position Overview

An exciting opportunity has emerged for a dedicated and skilled pharmacist with outstanding clinical and communication abilities to spearhead and provide exceptional service to the Integrated Care Operations (ICO) at University College London Hospitals.

The ICO manages a variety of services aimed at ensuring safe and effective transitions of care. These services include:

  • A home nursing initiative that facilitates safe discharges for patients under the supervision of a UCLH Consultant.
  • The Integrated Discharge Service (IDS), which collaborates closely with North Central London (NCL) Integrated Care Services to offer an Admission Avoidance service to the Emergency Department through pathways such as Camden and Islington Virtual Wards, along with specialized services like the Camden Adult Pathway partnership (CAPP), a Homeless Intermediate Care service, and a Rough Sleeper Nurse working with local outreach teams.

The successful candidate will be an integral part of the Medicine and Emergency Services pharmacy team, delivering pharmacy services and ensuring safe transitions of care between the ICO and other NCL Integrated Discharge Services.

Main Responsibilities

Key responsibilities will encompass the coordination and active participation in the daily clinical pharmacy services for the ICO team, as well as attending relevant meetings and handovers. You will collaborate with the ICO team on projects aimed at developing, implementing, maintaining, and enhancing new services.

Utilizing your independent prescribing expertise, you will optimize medication regimens in preparation for patient discharges and serve as a resource for staff and patients regarding medication inquiries.

As a member of the multidisciplinary team (MDT), you will engage in audits, service enhancement, and quality improvement initiatives related to existing systems and processes.

Additional leadership and management duties for the ICO and ESD will include governance, analysis and reporting of medication expenditures, conducting audits, and evaluating services, as well as providing education and training within the MDT.

Within the pharmacy department, you will contribute to both local and national pharmacy initiatives and support the delivery of educational and developmental programs.
