Join the dynamic clinical pharmacy team at Exeter City Primary Care Network, where we are committed to enhancing patient care through innovative pharmacy services.

The successful candidate will collaborate within a multi-disciplinary team, utilizing their expertise in clinical medication management to contribute to the pharmacy services offered to the network. This role aligns with the objectives outlined in the NHS Long Term Plan and the national PCN Direct Enhanced Services.

As part of your professional development, you will be encouraged to undertake the national CPPE Primary Care Pharmacy Education Pathway, with support provided to help you thrive in this position, even if you are new to the field.

In this role, you will work alongside a dedicated team of pharmacists and pharmacy technicians, ensuring safe and effective prescribing for our patient population.

Key Responsibilities

We are seeking a pharmacy technician who is passionate about patient care and eager to enhance their skills in medication management, particularly in support of our prescribing initiatives.

Your responsibilities will include:

  • Assisting clinical pharmacists in conducting medication reviews and ensuring accurate medicines reconciliation for patients transitioning between care settings.
  • Collaborating with local community pharmacists to enhance patient care.
  • Performing medicines reconciliation following patient discharge and during other transitions of care.
  • Executing tasks related to medication optimization, including checking inhaler techniques and engaging with patients to ensure effective medication use.
  • Managing prescription queries within your area of expertise.
  • Providing training and development for practice-based prescribing clerks and administrative staff to improve pharmacy operations within general practice.
  • Reporting on key performance indicators to demonstrate the value added to patients and general practice, in line with NICE guidelines.
  • Maintaining relevant information reporting systems and databases, including incident reporting and patient outcomes.
  • Liaising with pharmacy stakeholders, including the Medicines Optimisation team and community pharmacy.
  • Handling sensitive and confidential information in accordance with data protection regulations.
  • Organizing and planning your workload to meet established deadlines.
  • Utilizing clinical systems for conducting audits and ensuring compliance with medication alerts.
  • Building and maintaining effective working relationships with GP practices and other healthcare professionals.
