Key Responsibilities

The ideal candidate will engage directly with patients to manage chronic conditions, whether in a clinical setting, at home, or in residential facilities. Strong written and verbal communication skills are essential, along with a background in conducting comprehensive clinical medication assessments. The successful candidate will collaborate with a multidisciplinary team to establish and refine processes for effective medication management and safe prescribing practices. Experience in a General Practice environment is beneficial but not mandatory if the candidate possesses current clinical knowledge and understands the medication review process in primary care.

Core Duties and Responsibilities

Role Purpose

The post holder will operate within their clinical expertise as part of a multidisciplinary team, providing specialized knowledge in clinical medication management, conducting structured medication reviews, managing chronic conditions, overseeing medication during transitions of care, and supporting safe prescribing systems while addressing public health and social care needs.

The role includes performing face-to-face medication assessments for patients with complex medication regimens, particularly for the elderly, those in care facilities, and individuals with multiple health issues. The post holder will lead quality improvement initiatives and clinical audits while managing aspects of the Quality and Outcomes Framework.

This position is crucial for enhancing care quality and operational efficiency, requiring a commitment to delivering exceptional service within General Practice.

The post holder will receive support to advance their role towards becoming a non-medical prescriber if they do not already hold this qualification.

Key Duties

  • Conduct clinical medication assessments and provide recommendations to Senior Clinical Pharmacists, Nurses, and GPs regarding prescribing and monitoring.
  • Perform medication reviews in care homes, collaborating with staff to enhance the safety of medication management.
  • Manage clinics for long-term conditions, ensuring optimal medication use and monitoring for patients with various medical issues.
  • Engage in domiciliary medication reviews and participate in multidisciplinary case discussions.
  • Identify patient cohorts at high risk of medication-related harm through systematic practice computer searches.
  • Analyze medication use associated with unplanned hospital admissions and implement strategies to mitigate risks.
  • Support patients with common ailments while working within defined competencies and referring to appropriate healthcare professionals as necessary.
  • Provide patient-facing support for medication inquiries and ensure continuity of care post-hospital discharge.
  • Deliver pharmaceutical information to practice staff and patients, addressing relevant medication-related queries.
  • Contribute to the repeat prescribing policy and manage the reauthorization process, ensuring appropriate monitoring is in place.
  • Engage in collaborative working with various stakeholders to enhance patient care.
