About the Role

We are seeking an experienced and skilled Patient Pathway Coordinator to join our team at Homerton Healthcare NHS Foundation Trust. As a key member of our Medical Day Unit, you will play a vital role in ensuring the smooth flow of patients through our services.

Key Responsibilities
  • Monitor and review patient care and treatment progress using our Patient Tracking List, adhering to Trust and local policies.
  • Receive patient referrals and administer them in line with timings and requirements set out in Trust and National policies.
  • Coordinate and provide administrative support to relevant teams to ensure all relevant paperwork and clinical information is available.
  • Evaluate and escalate any issues and breaches of Waiting Times standards to relevant management in line with agreed escalation procedures.
  • Ensure timely and efficient transfer of patients and information between trusts/other organizations involved in the patient pathway, adhering to agreed communication pathways and protocols.
  • Ensure all health records are appropriately tracked and securely stored while in the department, according to Health Records Policy.
  • Responsible for collecting and updating patient information on relevant databases in line with local policy, to ensure efficient pathway progress.
  • Review and support development and implementation of Standard Operating Procedures.
  • Provide support for day-to-day queries from administrators.
  • Undertake any training as required to complete tasks associated with the job role.
