We are seeking a highly organized and detail-oriented Patient Pathway Coordinator to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our clinical support team, you will play a vital role in ensuring the smooth operation of patient care pathways across our hospitals.
Key Responsibilities- Monitor and Review Patient Care: Collaborate with clinicians to review patient care and treatment progress, ensuring adherence to Trust and local policies.
- Manage Patient Pathways: Effectively manage patient pathways, liaising with clinicians as necessary to ensure timely and efficient care.
- Data Management: Ensure accurate data input and maintain the accuracy of patient information on relevant databases.
- Administrative Support: Provide administrative support to relevant teams, ensuring all necessary paperwork and clinical information is available.
- Communication and Collaboration: Develop working relationships with clinical and managerial staff, ensuring effective communication and collaboration across teams.
- Quality Improvement: Identify areas for improvement and suggest solutions to enhance patient care pathways and tracking processes.
- Departmental Development: Participate in departmental development and contribute to the delivery of high-quality patient care.
