Since the integration of Children's cardiac, respiratory, and intensive care services, we have established a unified clinical directorate across our sites, fostering collaborative efforts between teams to enhance patient care.

Primary Duties

- Monitor and assess patient care and treatment progress in accordance with Trust and local policies.

- Adhere to local protocols for managing patient pathways and collaborate with clinicians as necessary.

- Ensure precise data entry and management.

- Identify subsequent steps in the patient care pathway based on clinical requirements.

- Process patient referrals in compliance with established timelines and requirements.

- Provide administrative support to relevant teams, ensuring all necessary documentation and clinical information is readily available.

- Respond to inquiries regarding patient care and treatment pathways from external Trusts and General Practitioners promptly.

- Develop a comprehensive understanding of the milestones within the designated patient pathways.

- Attend relevant meetings and execute actions concerning patient care within the designated pathways.

- Participate in national audits and collect data as required to facilitate the Trust's efforts in enhancing treatment for patient cohorts.

- Propose and identify improvements to the patient care pathway and tracking processes.

- Proactively monitor patients on their pathways and collaborate with clinicians and managers to expedite treatment.

- Address daily operational issues, ensuring resolutions are implemented and escalate significant concerns to management as appropriate.

- Facilitate the timely transfer of patients and information between trusts and other organizations involved in the patient pathway, adhering to established communication protocols.

- Collaborate within a multi-disciplinary team to develop services in alignment with departmental plans and Trust objectives.

- Ensure that all health records are accurately tracked and securely stored in accordance with Health Records Policy.

- Cultivate professional relationships with a diverse range of clinical and managerial staff.

- Responsible for collecting and updating patient information in relevant databases, ensuring efficient pathway progression and data accuracy.

- Undertake administrative tasks as required to support the delivery of high-quality patient care.

- Actively engage in departmental development initiatives and participate in meetings.

- Perform any other duties necessary to ensure the smooth operation of the department.

- Complete any required training to fulfill the responsibilities associated with the role.

Qualifications and Experience

Essential

  • GCSEs or equivalent, including Mathematics and English.

Desirable

  • Customer service qualification or training.

Experience

  • Proven experience in an administrative capacity.

  • Experience in an administrative role within the NHS.
  • Experience in supervising staff.
