Position Overview

We are looking for a motivated individual to join our team as a Patient Journey Coordinator within the General Surgery Secretaries, focusing on the Breast department.

Key Responsibilities

1. Develop a comprehensive understanding of the trust's data management processes, enhancing skills to rectify any discrepancies in the hospital's internal databases.

2. The incumbent will be tasked with liaising with patients and their families regarding appointment and admission information, both verbally and in written form.

3. Assist in analyzing capacity and demand to pinpoint limitations and opportunities for enhancement.

4. Support the validation and performance oversight of Patient Tracking List (PTL) monitoring, ensuring that booking, data entry, clinic, and elective admission outcomes are actively managed to meet established targets.

5. Report to the Service Support Manager, providing a user-centered administrative service to the General Surgery Team.

6. Ensure effective monitoring of patient progress along the care pathway and adherence to waiting time objectives.

7. Provide coverage within the team as necessary.

8. Adapt working hours as required to meet service demands.

9. Ensure compliance with the Trust's waiting list policy in alignment with national targets.

10. Implement and adhere to service modifications as needed.

Working Environment

The successful candidate will embody the values and behaviors expected by the Directorate and the Trust, as outlined in the performance management and personal development documentation. All staff are expected to comply with relevant policies and codes of conduct.

The primary objective of this role is to deliver exceptional service to the Trust, and the job holder is expected to achieve this through personal accountability and adherence to key characteristics.

Candidate Profile

Qualifications and Experience

Essential Criteria

Minimum qualifications of GCSE or equivalent Level 2 in Literacy and Numeracy. Proof of Mathematics and English qualifications. Proficient in Microsoft Office applications including Outlook, Word, and Excel.

Desirable Criteria

Familiarity with NHS software, policies, and procedures. Business Administration Level 2 qualification. Microsoft Office IT certification. Experience working in an NHS environment.

Skills and Abilities
