About the Role

We are seeking a highly skilled and motivated Advanced Clinical Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our UCR and virtual ward team, you will play a vital role in delivering high-quality care to patients and preventing avoidable hospital admissions.

Key Responsibilities
  • Work closely with an advancing practice team to deliver clinical practice and support trainee ACPs in developing their expertise.
  • Take a lead role in clinical assessment of patients with complex acute and/or chronic disease presentations, making differential diagnoses and prescribing treatments as an independent prescriber.
  • Lead clinical responsibility for patient caseloads and support the development of staff potential.
  • Collaborate with Consultant Practitioners and Trainee Consultant Practitioners to establish values-based professional expertise and evidence-based practice across pathways, services, and systems.
