Job Summary

We are seeking a highly motivated and experienced Advanced Clinical Practitioner to join our team focused on preventing avoidable hospital admissions. The successful candidate will work closely with an expanding practice team, taking a leading role in the clinical assessment of patients presenting with complex acute or chronic conditions.

Key Responsibilities

  • Provide independent clinical assessments and prescribe treatments for patients with diverse healthcare needs.
  • Collaborate closely with individuals, families, and carers to develop personalized care plans.
  • Lead and clinically supervise trainee Advanced Clinical Practitioners, fostering their professional development.
  • Work in partnership with the Consultant Practitioner to establish evidence-based best practices across all clinical pathways.

Qualifications and Skills

The ideal candidate will possess:

  • Appropriate professional qualifications and UK registration.
  • An Advanced Clinical Practice MSc degree.
  • Independent non-medical prescribing or pharmacology qualification.
  • Teaching and assessing qualification.
  • Excellent communication skills and a strong command of the English language.
  • Proficiency in computer literacy for data entry and electronic communication.
