Job Summary

We are seeking a highly motivated and experienced Advanced Clinical Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our UCR and virtual ward team, you will play a vital role in delivering high-quality care to patients and contributing to the prevention of avoidable hospital admissions.

Main Responsibilities

The successful candidate will be responsible for:

  • Working closely with an advancing practice team to deliver clinical practice and take a lead with clinical responsibility for patient caseloads.
  • Supporting trainee ACPs to develop their four pillars of practice, including clinical assessment, prescribing, and leadership.
  • Leading clinical assessments of patients with complex acute and/or chronic disease presentations, making differential diagnoses, and prescribing treatments as an independent prescriber.
  • Working collaboratively with the Consultant Practitioner and Trainee Consultant Practitioner to establish values-based professional expert and evidence-based practice across pathways, services, and systems.
  • Providing values-based leadership across the care pathway, services, and systems in complex and changing situations.

Person Specification

The ideal candidate will possess:

  • An Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice.
  • Professional UK registration with the NMC or HCPC.
  • Independent non-medical prescribing or pharmacology level 7 profession-relevant qualification.
  • Teaching and Assessing qualification or equivalent/mentorship qualification.
  • Able to demonstrate a good command of the English Language both written and verbal.
  • Computer Literate in both data entry and communicating via IT - emails and reports.
