About the Role

We are seeking an experienced and skilled Community Wound Care Nurse to join our team at Accelerate CIC. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality wound and lymphoedema care to our patients in the community.

Key Responsibilities
  • Provide skilled care and management to patients with acute and hard-to-heal wounds, including lower limb leg ulcerations and lymphoedema.
  • Work collaboratively with the clinical team to develop and implement individualized care plans that promote patient-centered care and improve outcomes.
  • Contribute to the development of innovative community-based solutions and provide education to peers and patients on wound and lymphoedema management.
  • Participate in quality improvement initiatives and contribute to the achievement of key performance indicators (KPIs) and internal audit.
Requirements
  • First-level registered nurse with current NMC registration or equivalent.
  • Minimum 1 year's nursing experience post-qualification, with clinical experience of patients with long-term conditions, wound assessment and management, and lower limb leg ulcerations and general lymphoedema management.
  • Experience of working within a team and knowledge of professional issues in healthcare.
