We are seeking an experienced and skilled Community Wound Care Nurse to join our team at Accelerate CIC. As a Community Wound Care Nurse, you will play a vital role in delivering high-quality wound care services to our patients in Tower Hamlets and City and Hackney.
Key Responsibilities- Provide skilled care and management to patients with acute and hard-to-heal wounds, including lower limb leg ulcerations and general lymphoedema management.
- Work collaboratively with the Clinical Lead and other team members to deliver innovative community-based solutions and provide an excellent and structured development programme.
- Contribute to the development of a cohesive team, supporting the Clinical Lead in managing team objectives and remaining mission-focused.
- Be accountable for providing daily management within the leg ulcer clinics and homes across the borough, ensuring effective management of team objectives and remaining mission-focused.
- Encourage individualized care that enables people to tolerate treatments and thereby improve outcomes.
- Enhance the delivery of Quality improvement initiatives, KPIs, and internal audit.
- Bring critique to internal and external systems, creating positive change through effective team working.
- First-level Registered Nurse with current NMC Registration or Equivalent.
- Minimum 1 year's nursing experience post-qualification, with clinical experience of patients with long-term conditions.
- Experience of wound assessment and management, including lower limb leg ulcerations and general lymphoedema management.
- Experience and knowledge of a variety of compression bandaging.
- Experience of working within a team and knowledge of professional issues in healthcare.
