Reablement Service Assistant

**Job Summary**
We are seeking a skilled and compassionate Care and Support Worker to join our Reablement Service team at Wakefield Council. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals in their own homes, helping them to regain independence and confidence.
**Key Responsibilities**
- Deliver person-centered care and support to individuals with disabilities and those recovering from illness or injury
- Work collaboratively with colleagues and other healthcare professionals to develop and implement personalized care plans
- Provide short-term care and support to individuals in their own homes, including bridging services and emergency response
- Contribute to the development and implementation of care and support plans, ensuring that individuals receive the best possible outcomes
- Participate in ongoing training and professional development to enhance skills and knowledge
**What We Offer**
- A competitive salary of £21,000 - £21,000 (FTE) or £11.38 per hour
- Enhancements for weekend and bank holiday working, plus overtime opportunities
- 26 days holiday per year
