About the Role

As a Virtual Ward Nurse, you will have the opportunity to work in a dynamic and supportive environment, with access to ongoing training and development opportunities.

Main Responsibilities
  • Assess, plan, implement, and evaluate holistic care for patients in their own homes.
  • Develop integrated care plans in partnership with patients, families, and other healthcare professionals.
  • Order equipment and refer patients to other services as necessary.
  • Supervise and develop junior staff, including Health Care Assistants and Students.
  • Work collaboratively as part of a multi-professional team to provide high-quality care to patients.
  • Undertake a range of competent clinical interventions and identify any deterioration in patients' condition, taking appropriate action.
  • Manage a caseload of patients with complex needs, associated with a specific specialty.
  • Support in duty role and complete triages.

Person Specification
  • Essential:
    • Diploma or Degree in Adult Nursing, registered as fit to practice by the Nursing and Midwifery Council (NMC).
    • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
    • Supporting others and mentorship, insight and understanding of current issues in nursing and the NHS.
  • Desirable:
    • Mentoring qualification equivalent.
    • Community experience and working as an autonomous practitioner.
    • Experience with the use of Microsoft Office and a variety of IT packages.
Skills and Abilities
  • Essential:
    • Excellent communication skills, good interpersonal skills, and a commitment to high standards of patient-centred care.
    • Adaptable and flexible, effective organisational skills, and the ability to reflect on practice, accept criticism, and act constructively.
    • Understanding of own accountability, responsible, and reliable, utilises a problem-solving approach to work.
    • General IT skills and the ability to learn new technology quickly, holds a current UK driving licence/permit and access to a vehicle.
