Job Description

Windmill Surgery is seeking a highly skilled and experienced Advanced Practice Nurse to join our team. As a key member of our clinical staff, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our practice.

Key Responsibilities
  1. Chronic Disease Management: Develop, implement, and embed effective care management programs for patients with chronic conditions, ensuring evidence-based practice and optimal patient outcomes.
  2. Health Promotion and Wellbeing: Design and deliver health promotion and wellbeing programs to support patients in maintaining healthy lifestyles and preventing disease.
  3. Clinical Leadership: Provide clinical leadership and guidance to nursing staff, ensuring that they have the necessary skills and knowledge to deliver high-quality patient care.
  4. Medication Management: Review patient medications to enhance compliance and provide guidance on the use of prescribed and over-the-counter medications.
  5. Travel Medicine Services: Coordinate the provision of travel medicine services, including vaccinations and travel advice.
  6. Pathology Services: Request pathology services and process pathology results as required.
  7. Clinical Records: Maintain accurate and up-to-date clinical records, ensuring compliance with relevant legislation and guidelines.
  8. Team Collaboration: Work collaboratively with other healthcare professionals to ensure seamless patient care and effective communication.
  9. Professional Development: Stay up-to-date with the latest clinical guidelines and best practices, sharing knowledge and expertise with colleagues to enhance patient care.
Requirements
  • Registered Nurse with a strong background in chronic disease management and health promotion.
  • Advanced practice qualifications and experience in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and other healthcare professionals.
  • Strong leadership and clinical skills, with the ability to provide guidance and support to nursing staff.
  • Ability to work autonomously and as part of a team, with a flexible approach to workload and priorities.
