Accountability: Accountable to the Chief Nursing and Clinical Patient Operations Officer

Compensation: Competitive hourly rate

The Enhanced Access service is designed to support Primary Care Networks (PCNs) across various regions, operating from local General Practices.

As an Advanced Practice Nurse within our Enhanced Access team, you will be expected to:

  • Deliver high-quality nursing care autonomously to patients.
  • Engage in routine health surveillance and screening initiatives.
  • Manage chronic health conditions, contraceptive services, wound care, immunizations, and vaccinations.
  • Contribute to the advancement of innovative nursing practices, providing insights on service enhancements based on your expertise and understanding of patient needs.
  • Perform General Practice Nursing responsibilities on behalf of the patients and the PCN.

Key Responsibilities:

Clinical Duties:

  • Collaborate within a multidisciplinary team to achieve shared objectives.
  • Provide family planning consultations.
  • Administer intramuscular contraceptive injections in accordance with established protocols.
  • Deliver childhood vaccinations following the national immunization schedule.
  • Monitor ongoing treatment plans.
  • Offer health education and advice.
  • Conduct cervical cytology screenings.
  • Provide opportunistic health screenings and guidance on patient wellness.
  • Assist in the planning and evaluation of patient care.
  • Perform clinical measurements, including blood pressure assessments, blood glucose monitoring, and ECGs.
  • Administer subcutaneous and intramuscular injections.
  • Manage wound care and appropriate treatment of injuries.
  • Promote health in areas such as weight management, smoking cessation, and alcohol use.
  • Educate patients and their families on self-care practices as appropriate.
  • Support and/or accompany the general practitioner during consultations.
  • Maintain professional standards and stay informed about evidence-based practices.
  • Ensure proper registration with the relevant professional body.
  • Engage fully in the annual appraisal process and pursue personal professional development.
  • Participate in clinical supervision activities.
  • Uphold cleanliness and hygiene standards in line with infection control protocols.
  • Assist in the maintenance of medical equipment, ensuring regular checks and calibration, and report any faults as per policy.
  • Support the management of stock items in clinical areas, adhering to cold chain policies.

Qualifications:

Essential:

  • Relevant nursing or health-related degree/qualification
  • Registered first-level nurse
  • Registered with the Nursing and Midwifery Council (NMC)

Desirable:

  • Non-Medical Prescribing Qualification or willingness to pursue

Experience:

Essential:

  • Post-registration nursing experience
  • Experience in community or primary care settings
  • Management of chronic health conditions
  • Familiarity with clinical protocols and guidelines
  • Experience in audit processes

Desirable:

  • Involvement in quality initiatives such as clinical benchmarking
