Job Summary

Windmill Surgery is seeking an experienced and skilled Practice Nurse to join our dynamic team. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and supporting the delivery of policy and procedures.

Main Responsibilities
  • Support patients with the management of their asthma and COPD conditions, providing education and guidance on self-management techniques.
  • Work collaboratively with the general practice team to meet the needs of patients, ensuring the provision of healthcare is appropriate and evidence-based.
  • Develop and implement effective care management programs for frail and housebound patients, ensuring their needs are met and their health and wellbeing are promoted.
  • Assess patient needs, providing healthcare that is tailored to their individual requirements and promoting health and wellbeing.
  • Develop and implement health promotion and wellbeing programs, working to prevent adverse effects on patient health.
  • Provide routine nursing care to patients, diagnosing and managing acute and chronic conditions as required.
  • Prescribe medication for patients in accordance with PGDs and own scope of practice, reviewing patient medications to enhance compliance.
  • Provide guidance to patients on the use of prescribed and over-the-counter medications, ensuring they understand side effects and dosages.
  • Coordinate the provision of travel medicine services, requesting pathology services and processing pathology results as required.
  • Oversee the provision of chronic disease clinics, delivering patient care as necessary and referring patients to secondary/specialist care as required.
  • Maintain accurate clinical records, ensuring read codes are used effectively by all members of the nursing team and ensuring the appropriate maintenance and use of chronic disease registers.

  • Registered Nurse
  • Postgraduate diploma or degree in Asthma and COPD
  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working autonomously
