Job Summary

Windmill Surgery is seeking an experienced and skilled Practice Nurse to join our dynamic team. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and supporting the delivery of our practice's policies and procedures.

Main Responsibilities
  • Support patients with the management of their asthma and COPD conditions, providing education and guidance on self-management techniques.
  • Work collaboratively with the general practice team to identify and manage patients at risk of developing long-term conditions, promoting health and wellbeing.
  • Develop and implement effective care management programs for frail and housebound patients, ensuring the provision of appropriate healthcare services.
  • Assess patient needs, providing evidence-based practice and incorporating health promotion and wellbeing programs.
  • Prescribe medication for patients in accordance with PGDs and own scope of practice, reviewing patient medications to enhance compliance.
  • Provide guidance to patients on the use of prescribed and over-the-counter medications, including side effects and dosages.
  • Coordinate the provision of travel medicine services, requesting pathology services and processing pathology results as required.
  • Oversee the provision of chronic disease clinics, delivering patient care as necessary and referring patients to secondary/specialist care as required.
  • Maintain accurate clinical records, ensuring the use of read codes and the maintenance of chronic disease registers.
