Lead Nurse Clinician

Windmill Surgery is seeking a highly skilled and experienced Advanced Practice Nurse to join our team. As a key member of our clinical staff, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our practice.
Key Responsibilities- Chronic Disease Management: Develop and implement effective care management programs for patients with chronic conditions, ensuring evidence-based practice and optimal patient outcomes.
- Health Promotion and Wellbeing: Design and deliver health promotion and wellbeing programs to support patients in maintaining healthy lifestyles and preventing disease.
- Clinical Leadership: Provide clinical leadership and guidance to nursing staff, ensuring high standards of patient care and adherence to best practice.
- Medication Management: Review and optimize patient medications to enhance compliance and improve health outcomes.
- Travel Medicine: Coordinate the provision of travel medicine services, including vaccinations and travel advice.
- Pathology Services: Request and process pathology results, ensuring timely and accurate diagnosis and treatment.
- Clinical Records: Maintain accurate and up-to-date clinical records, adhering to relevant legislation and guidelines.
- Team Collaboration: Work collaboratively with other healthcare professionals to ensure seamless patient care and effective communication.
- Professional Development: Stay up-to-date with the latest clinical developments and best practices, applying this knowledge to improve patient care and outcomes.
- Registered Nurse with advanced practice qualifications and experience.
