About the Role

We are seeking a highly skilled and experienced Children's Community Nurse to join our team on a fixed-term basis. As a key member of our Ambulatory Model, you will play a vital role in providing high-quality care to children and young people with acute and long-term conditions.

Key Responsibilities:

  1. Provide skilled paediatric care to children and young people in the community, working autonomously and as part of a multi-disciplinary team.
  2. Develop and implement individualized care plans to meet the unique needs of each child and family.
  3. Collaborate with other healthcare professionals to ensure seamless care transitions and optimal health outcomes.
  4. Conduct nurse-led clinics to provide accessible and convenient care to children and young people with non-acute conditions.
  5. Participate in the development and implementation of policies and procedures to ensure the highest standards of care and safety.

Requirements:

  • Registered Children's Nurse with extensive clinical knowledge and skills in caring for children and young people with a variety of needs.
  • Experience of working in the community setting, with a strong understanding of the complexities of community-based care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with children, young people, and their families.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high standards of care.
