Job Description:

At Doylestown Health, we are seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality care for our patients.

Key Responsibilities:
  • Implement and maintain quality standards and guidelines to achieve recognition and benchmark primary care.
  • Identify, educate, and support team members on proper documentation processes and quality recognition.
  • Coordinate optimal transitions of care through communication with patients, caregivers, clinicians, and facilities.
  • Communicate with and schedule appointments according to care standards, identify and close care gaps, and provide support and education on self-management of chronic illnesses.
  • Facilitate team meetings with physicians and clinical staff to discuss high-risk patients and enhance the role of medical assistants and other clinical staff.
Requirements:
  • Education: High school diploma, medical assistant diploma from an accredited vocational institution, or a community college course in medical assisting.
  • Experience: Minimum 3 years of recent experience working in a medical facility as a medical assistant, with primary care office experience preferred.
  • Skills: Microsoft Office experience, current health records with appropriate immunizations, and knowledge of healthcare field and medical office protocols/procedures.
