About the Role
We are seeking a highly skilled and motivated Community Health Specialist to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Rapid Response and Intermediate Care service, you will play a vital role in providing expert clinical interventions and rehabilitation to patients in their own homes.
Your Mission
- To provide high-quality, patient-centered care that empowers recovery and prevents unnecessary hospitalization.
- To work collaboratively with a multidisciplinary team to deliver effective care and support to patients and their families.
- To contribute to the development of clinical pathways and protocols that promote best practice and improve patient outcomes.

Key Responsibilities

- To collaborate with other healthcare professionals to ensure that patients receive seamless and coordinated care.
- To contribute to the development of clinical guidelines and protocols that promote best practice and improve patient outcomes.
